Alternatively, you can use the self-referral form to send an email to the practice. As we are a private practice, you can self-refer to our service. This means you do not need a referral from a doctor.

All new referrals are discussed in confidence within the practice, in order to ensure best fit between client and clinician.

Shelly Beach Practice is closed over the Christmas period. All referrals submitted from 18th December 2020 onwards will be triaged on Thursday the 28th of January 2021. A clinician will then respond to your enquiry shortly after that date.
